Just another controller's button settings.
This file changes your controller layout to one that is similar to that of the Souls games and generally feels more intuitive in my opinion.

Install
Download the file and replace your C:/Documents/Files/The Witcher 3/input.settings file with the mod file.
To uninstall it, just delete input.settings and launch the game. It will re-download a new input.settings file.
