There are not even mission failures.

All missions completed (I'm sure I've completed them all, but I may have missed some random encounters, which I can't determine)
Completed the entire game on version 1.31 from the beginning so there are no map bugs, mission points
Used several mods

Install:

Unzip the folder and copy the SAV files to your Witcher 3 gamesaves folder.

Standard: C:>Users>Username>Files>The Witcher 3>Game

choose:

Bring Ciri to Emhyr
Didn't take money from Emhyr
Everyone helps fight the wild hunt in Kaer Morhen, including Letho
Support her every time (she becomes the queen ending)
Reject Triss and fall in love with Yen
Gave Keira Alexander's note and send her to Kear morhen
Godling Sarah ends up with Johnny
Kill Radovid, kill Dijkstra
Reject Triss and fall in love with Yen
Geralt survives Whoreson Junior
Cerys becomes the new king of Skellige
Didn't take money from Emhyr and got a new Roach (this one seems a bit faster than the standard one)
Everyone helps fight the wild hunt in Kaer Morhen, including Letho
Killed the spirit in the heart of the tree (Baron and his wife live)
Help Triss escape from the mage
Geralt uses Einar's fangs to break the curse. in spite of

It was Stoneheart's choice, and Geralt later killed Morkvarg.

Taking out the rose (Third Wish) - o'dim took away olgierd's soul

Unzip the archive from playbuff and read the bundled readme / instructions first.
