Place the save file into This PC>Documents>My Games>Borderlands 2>WillowGame>SaveData>76561198028988149
Then load your game and press "Axton The Head Whore" to start. Once you get past CL4P-TP's small cutscene, open your inventory and use all the head skins.
I used Gibbed's Borderlands 2 Save Editor (revision 256). Thanks to FallenEclipse on YouTube (I simply put them all in a save file. I can also make one with a skin if needed.
